An impersonal process and objective assessment

The building in Uspensky Lane was opened in May after renovation. Visitors are greeted by friendly staff at the reception, soft comfortable furniture and plenty of natural light throughout. On the walls are photographs from the real life of doctors.

Residents are sitting on the sofas; today they are being accredited. At the entrance they were given individual numbers, route sheets and key cards. Even the experts who take the exam do not know the personal data of the residents.

“We have a truly independent platform and an objective assessment. It is not influenced, for example, by the relationship with the teacher. Among the experts are chief doctors, their deputies, leading specialists. These are all medical practitioners with extensive experience. We have had cases when, after successfully passing all stages of accreditation, experts invited residents to work, ”clarifies Yulia Uvarova, Acting Head of the Moscow Center for Accreditation and Professional Development in Healthcare.

First, young specialists will have to pass the theory in the computer room. Then they move on to demonstrating practical skills: they change into medical uniforms and end up at the stations – these are rooms with simulation equipment. There are 31 stations in the center, including modular ones with mobile partitions. Residents can get inside by holding the key card to the screen near the door. After that, a task appears on the screen that needs to be completed.

On average, each resident goes through five stations, each takes 8.5 minutes (as approved by the Russian Ministry of Health). All young doctors show that they can provide emergency medical care and perform cardiopulmonary resuscitation. The rest of the tasks depend on the specialty. For example, future endocrinologists examine a patient who may have osteoporosis, and surgeons close the wound and perform planned and emergency operations.

“I needed to drain the pleural cavity, that is, to help release the air that had accumulated in the chest. In general, it is convenient to be accredited at the personnel center. I finished my residency last year and am already working in a private clinic and city polyclinic number 68, ”says resident Yulia Ermolaeva. She had just finished her assignment at one of the stations.

The experts behind the glass

Experts monitor the quality of practical assignments by observing residents from behind glass or studying broadcasts from CCTV cameras. They sit in headphones and fill out an exam checklist in a special program. And residents do not see the experts at all, because for them the glass is mirrored. They can only hear them if the task requires interaction.

“On the basis of the personnel center, residents in many specialties can be assessed, this is the uniqueness of the site. And we can evaluate in practice how much a person possesses the technique of performing certain interventions. But the residents are, of course, worried. They have no feedback: they cannot understand whether the expert approves of their actions or not. From experience I can say that those who train in advance and study the procedure pass. It may not work the first time, ”says Alexander Kalinichenko, a surgeon at the V.V. Vinogradov, who monitors the assignment.

By the way, you can also sign up for a practice exam. It is free and available to all residents who have applied for accreditation at the center. Such a rehearsal will multiply the chances of successful accreditation.

There are also nurses and nurses working in the center. They attend some of the exams and act as an assistant. Patients also come to the rescue. They communicate with the doctor when he is performing the task of collecting anamnesis: answering questions and telling what worries them.

Get real skills on the simulator

The center’s equipment park includes more than 1,100 units of the latest simulation and medical equipment. It helps to conduct professional assessments in 88 specialties.

So, for neonatologists there is a station with dummies of babies, including premature babies. And in the next office you can see Peter – such a nickname was given by the specialists of the center to a man’s mannequin, which is as close as possible to a living person. “He is like a real person, he has bristles, eyelashes, veins, and the weight and height of the mannequin fully correspond to the characteristics of an adult man,” says Sergey Sergeev, a leading specialist in simulation equipment. Such dummies, in particular, help to practice the skills of caring for bedridden patients.

At the next station there is a SimMan 3G dummy. He visually does not look like a person, but is able to imitate almost all possible reactions of a living organism. These robots blink and breathe, sweat and scream, and experience convulsions. These reactions bring the resident closer to real stressful conditions that may arise during work.

“The simulation equipment certainly doesn’t compare to reality. But of all the mannequins that I have seen and could appreciate, this is probably the best. He even inflates veins. After certification, I would like to go to an emergency hospital. While I am young, I feel that I am ready, ”says Evgeny Iskander, a graduate of the residency in radiology at the N.V. Sklifosovsky.

The personnel center also has equipment for dentists. You can drill the teeth of a mannequin to get rid of tooth decay. And on the simulator of endoscopic interventions (GI Mentor), they train to do bronchoscopy, gastroscopy and colonoscopy. During operation, real mucous membranes of a person are displayed on the screen of the device. And for example, if the endoscope is inaccurately inserted during gastroscopy, the device will begin to cough.

Several devices have been installed for surgeons, including those for minimally invasive interventions. There are also unique ones – Da Vinci and ANGIO Mentor. With the help of the latter – a hybrid operating room – it is possible to study in detail the pathologies and structure of real people who, after death, bequeathed their bodies to science.

“Before, this was not the case, after accreditation, residents immediately came to work. Now there is an opportunity to work out the technique on simulators, to learn basic skills. Laparoscopic technique was not available at all for a long time, now all residents can learn it. Of course, this is different, it is not living tissue, but such training is useful at the initial stage, ”notes Yuri Busyrev, Deputy Chief Physician for Surgery at the I.V. Davydovsky.

Prepare new and train experienced

At the third stage of certification, residents receive a specific task – they must make a diagnosis based on known data and prescribe treatment. Since the beginning of the work of the personnel center, more than 500 residents have been successfully accredited.

“We get a snapshot of the current knowledge that novice doctors have after all stages of training: medical school and residency. If we see that some knowledge is lacking (for example, the participants make the same mistakes), then we immediately understand how to react to this. For example, prepare an educational program or refresher courses. The site is also good because we can promptly respond to a request for training already experienced doctors, ”adds Yulia Uvarova.

So, in May – June, ultrasound specialists were trained here, who work in polyclinics, where major repairs were made according to the new Moscow standard. Doctors were trained to work on new equipment, which they purchased after the modernization of institutions. And in the future, the personnel center will become a single platform for assessing the professional skills of all doctors in the capital.
